


Claims Procedures
Prior authorization must be obtained by you before a repair or replacement of a covered tire and/or wheel. Call Claims at 1-877-778-5570 for a claim authorization number and a claim form. Mail all documents to the address listed on the claim form, within (30) days from the purchase date of the covered repairs or replacement of tire and/or wheel.
- Copy front and back of the Agreement.
- Claim form, completed and signed by the registered customer and representative of the dealership/service center.
- Copy of the replacement tire and/or wheel sales invoice which must document; A. the tread depth of the damaged tire. B. that the damaged tire and/or wheel was not repairable. C. that the damage was not due to a manufacturers defect.
- Copy of invoice for covered repairs and/or road service (not to exceed $50 per occurrence), if applicable.
- Copy of buyers order, insurance policy and service contract if applicable.
- Any other information reasonably requested.
Administrator reserves the right to inspect all damaged tires and/or wheels prior to making any claim payments. Any fraudulent claims will be prosecuted to the fullest extent of the law.

To obtain key replacement benefits under this agreement, member must comply with the following conditions:
1. All claims must be reported to Performance Automotive Management, LLC as soon as reasonably possible at 2310 E. Atlantic Blvd., Suite 205. Pompano Beach, Fl 33062 or phone 1-877-778-5570
2. If member is within a 25 mile radius of the originating dealer, Member must return there to have a replacement key cut and programmed. If member is outside the 25 mile radius, Member may go to the appropriate franchise dealer in his/her area.
3. All non-working keys or remotes must be made available to Dealer and Administrator for inspection.
4. The originating dealer must fax a copy of the original repair order to Administrator for payment.
5. The originating dealer will be paid as soon as appropriate repair order is received by Administrator from the originating dealer. The member is not responsible for any out of pocket expense other than costs in excess of their yearly limit of $400 on Plan A and $800 on Plan B.
6. If it is necessary for a member to go to non-originating dealer member shall call Administrator at 1-877-778-5570 for proper approval before replacing the key or remote. Member will pay non-originating dealer for the replacement key or remote and will be reimbursed upon receipt of all repair orders, sales invoices and/or other relevant or appropriate documentation as may be reasonable requested by Administrator.
7. Administrator is only agreeing to pay the replacement cost for eligible keys or remotes under the terms, conditions and limitations set forth in this agreement. Administrator shall not provide any keys or remotes itself. Further, Administrator does not in any way warrant or guarantee, whether express or implied, any replacement key or remote obtained by Member and/or paid for by Administrator.
IN THE EVENT OF A CLAIM, the Customer must provide, within 60 days of the Date of Loss, Performance Automotive Management at 2310 E. Atlantic Blvd., Suite 205, Pompano Beach, Florida 33062, 1-877-778-5570 the following documentation before any payment under this Addendum can be processed. Failure to provide within 90 days of the Date of Loss will VOID this Addendum.
* 1. Complete documentation from the Customer’s primary insurance company substantiating the date of and cause of loss of the Vehicle, gross settlement amount, deductible, the net settlement amount and proof of payment. Should the Customer not have collectible physical damage insurance on the Date of Loss, it is the Customer’s responsibility to advise Performance Automotive Management in writing immediately when the loss is discovered and Performance Automotive Management will calculate the Actual Cash Value of the Vehicle immediately prior to the loss.
- Copy of Retail Installment Sales Contract/Loan Agreement.
- Copy of GAP Addendum.
- Copy of Bill of Sale (Invoice) for the Purchase of the Vehicle substantiating Vehicle make, model, year, options, and accessories, any cancelable items purchased, total purchase price and amount financed.
- Copy of the police report (theft losses only).
- Documentation showing refund amounts of any cancelable items purchased.
PLEASE MAKE SURE all documents are totally legible, otherwise payment will be suspended until legible copies can be obtained.
IN THE EVENT OF A CLAIM, the customer must provide within 90 days of the Date of Loss, "Administrator" with all of the following documentation before any Warranty payment can be processed, failure to provide this documentation within the 90 day period will void this Warranty.
- Copy of Police Report with narrative.
- Copy of Customer Warranty.
- Buyers orders and finance agreement for original vehicle and/or replacement Vehicle.
PLEASE MAKE SURE that all the above documents are totally legible,
otherwise Warranty payment will suspend until legible copies can be
obtained A copy of Customer’s Primary Insurance Company Settlement
Check and Proof of Loss, substantiating the Date of Loss, specific Cause
of loss and Gross Settlement Figure. In the event the Customer does not
have physical damage theft insurance as of the Date of Loss, the
Customer must notify in writing immediately when the loss is discovered.
A loss adjuster will be instructed to in writing immediately when the
loss is discovered. A loss adjuster will be instructed to investigate
the circumstances of the theft and if it is determined when the loss is
covered, will calculate Actual Cash value of the Vehicle at Date of
Loss. All such loss adjusting fees will be the sole responsibility of
the Customer and will be deducted from any claim payment. In the event
the Loss is determined not to be covered, the Customer must reimburse
"Administrator" for the loss adjusting fees.

GAP
GAP stands for Guaranteed Asset Protection. This is coverage that provides financial security for customers. When a vehicle is purchased, the value of it immediately drops, causing the customer to owe more money on the car than it is actually worth. Therefore, if the vehicle is totaled, the potential exists for the insurance company to offer a settlement that does not even cover what is left to pay on the loan, creating a "gap." Performance GAP covers this difference.
Depending on the coverage, GAP does not cover commercial vehicles, motorcycles, boats or vehicles that are financed or have an MSRP over $100,000.

Key Replacement
Key Replacement Program Coverage:Thanks to technological advancements of the automotive industry most vehicles are operated with a programmable key and remote system.In the event your key/remote is lost, stolen or destroyed we will pay for a replacement key remote.

Tire & Wheel Protection Program Coverage:Covered Road Hazard: Potholes, rocks, wood debris, metal parts, plastic or composite scraps or any item causing tire, wheel or rim damage beyond normal wear and tear. Coverage Includes:- Repair or replacement coverage for tire and rims (including mounting, balancing, and taxes- Emergency road assistance benefit.
